1. Service

Evida Health provides software for clinical data abstraction, evidence review, registry reporting, accreditation reporting, and quality measure workflows.

The Service helps hospital quality teams organize source evidence, draft measure answers, review missing fields, and prepare submission-ready case records.

2. Accounts

You are responsible for maintaining accurate account information and protecting access to your account.

You must notify Evida Health if you suspect unauthorized access or misuse of the Service.

3. Clinical Review

Evida may generate suggested abstractions, evidence links, confidence indicators, and quality workflow outputs.

Users remain responsible for reviewing, validating, approving, correcting, and submitting any clinical, registry, accreditation, or quality reporting output.

Evida does not replace clinical judgment, registry program guidance, accreditation requirements, or hospital compliance review.

4. Customer Data

You retain ownership of the data, documents, and content you submit to the Service.

You are responsible for ensuring that you have the rights, authorizations, consents, and agreements required to upload or process data through the Service.

Evida Health processes customer data to provide, secure, support, and improve the Service according to applicable agreements and policies.

5. Security And Compliance

Evida is designed for healthcare workflows and uses administrative, technical, and organizational safeguards to protect customer data.

No system can be guaranteed to be completely secure. You are responsible for configuring access controls and using the Service according to your organization's policies.

6. Third-Party Systems

The Service may connect with EHRs, registries, identity providers, storage systems, and other third-party services.

Evida Health is not responsible for third-party system availability, data quality, program rule changes, or external submission requirements.

7. Intellectual Property

The Service, software, design, documentation, and related materials are owned by Evida Health or its licensors.

You may not copy, modify, reverse engineer, or use the Service outside the rights granted to you by Evida Health.

8. Termination

Evida Health may suspend or terminate access to the Service if these Terms are violated or if continued access creates security, legal, or operational risk.

You may stop using the Service at any time, subject to any separate written agreement with Evida Health.

9. Disclaimers

The Service is provided on an "as is" and "as available" basis except as expressly stated in a separate written agreement.

Evida Health does not warrant that generated outputs will be complete, error-free, accepted by a registry, or sufficient for any accreditation or compliance purpose without user review.
